Job Title: Intermediate Care Team Social Worker Location: Portsmouth, UK Pay rate: £35-38 per hour (depending on experience) Contract: Full-time, Locum Department: Adult Social Care – Intermediate Care Team About the Role: Portsmouth City Council is seeking a dedicated and proactive Social Worker to join our Intermediate Care Team. This service focuses on supporting adults during their recovery from illness, injury, or hospital discharge, helping them regain independence and avoid long-term care placements. You will play a key role in assessing individuals' needs and working with a range of professionals to ensure they receive the right support at the right time. Key Responsibilities: * Conduct assessments under the Care Act 2014 for individuals receiving intermediate care services. * Develop and review personalised care and support plans, ensuring they are tailored to each individual’s needs and promoting independence. * Work closely with health professionals, including occupational therapists, physiotherapists, and hospital discharge teams, to provide integrated care. * Assist with the transition from hospital to home or other care settings, ensuring smooth, safe, and timely discharges. * Monitor the progress of service users during their recovery and ensure care plans are adjusted as needed to reflect changes in health or circumstance. * Undertake safeguarding work to protect vulnerable adults from abuse and neglect, following statutory procedures. * Maintain accurate case records and ensure all assessments, plans, and reviews are completed to a high standard. * Support individuals, families, and carers by providing advice and guidance on services and entitlements. Essential Requirements: * Degree in Social Work (or equivalent qualification). * Registered with Social Work England. * Experience working in adult social care, preferably with experience in hospital discharge or intermediate care settings. * Sound knowledge of relevant legislation, including the Care Act 2014 and safeguarding policies. * Excellent communication, assessment, and decision-making skills. * Ability to work effectively within a multi-disciplinary team and manage a caseload in a fast-paced environment. * Full UK driving licence and access to a vehicle for community visits
