Social Worker - Transformation Learning Disabilities Team Middlesbrough, UK £29 - £37 per hour About the Role: Randstad is seeking a dedicated and experienced Social Worker to join the Transformation Learning Disabilities Team in Middlesbrough. This role offers an exciting opportunity to be part of a dynamic team focused on transforming services for individuals with learning disabilities, ensuring they receive the highest quality of care and support. Key Responsibilities: Conduct comprehensive assessments of individuals with learning disabilities to identify their needs and develop tailored support plans. Work collaboratively with a multidisciplinary team, including health professionals, educators, and families, to deliver person-centered care. Advocate for individuals with learning disabilities, ensuring their rights and wishes are at the forefront of all decisions. Facilitate and contribute to the development of innovative services and practices that enhance the lives of those with learning disabilities. Monitor and review support plans regularly to ensure they remain effective and responsive to changing needs. Keep accurate and up-to-date records in line with legal and organizational requirements. Requirements: Qualified Social Worker with a valid Social Work England registration. Proven experience working with individuals with learning disabilities. Strong understanding of current legislation and best practices related to learning disabilities. A commitment to promoting equality, diversity, and inclusion in all aspects of work.
