Job title: Home Care Assessor Hours: Monday to Sunday (working 5 out of 7 days) Salary: £26,000.00 (Plus on call allowance) Post: One full time post Location: Field Based
About the role As a Home Care Assessor, you will travel throughout Gloucestershire, (we currently cover Tewkesbury, Berkeley and Stroud), using one of our pool cars, visiting our clients and care workers, completing initial client assessments, and carrying out care plan reviews. You will be working alongside other healthcare professionals, as well as liaising with clients and their families. What you will be doing Carry out duties to enable BHC to continue to comply with, and meet all standards relevant to national regulation and local authority requirements.
Carry out the initial Care Plan assessments for new Service Users and assessment of needs.
Carry out follow up clients reviews.
Participate in rostered on call.
What we need from you Previous experience of Social Care, as Supervisor or Assessor.
Previous care role experience.
Be able to work under pressure, prioritise and stick to deadlines.
Have excellent judgement and decision-making skills.
Be flexible and able to sometimes work outside of normal working hours and occasionally work bank holidays and weekends.
Full driving licence and access to own vehicle is required for this position.
Good communication skills both verbally and written.
