Job Title: District Nurse Tripod Partners are currently recruiting for a District Nurse to join the Wembley District Nursing Team as a Clinical Team Lead. This is a full-time role providing leadership and clinical expertise within the community nursing service. Key Responsibilities * Lead the District Nursing Team, providing clinical supervision and guidance to ensure high-quality patient care. * Manage and coordinate the care of patients with complex health needs in the community, working closely with the multi-disciplinary team. * Conduct assessments, create care plans, and manage treatment for a caseload of patients, ensuring timely and effective care.
