Social Worker, Mental Health Team

Social Worker required for Sheffeild Council's Mental Health Team. Immediate start with ongoing contract available with competitive rates up to £30.00 per hour Responsibilities include: Support, care planning, mental health and CPA reviews Safeguarding Comprehensive needs assessment Risk assessment and management of mental health symptoms Ensure accurate and timely recording of all information/assessment activity onto the social care records database Be a skilled and reflective social work practitioner with demonstrable experience of working with severe mental illness in a community setting. Have knowledge and experience of working within as a care coordinator in a local authority or the NHS.
