We are looking for one Social Worker in our Pathway 1 Community Integrated Team.

The team is part of the hospital discharge to assess model, where service users are discharged home, from acute hospitals as part of this process. Once you are assigned, as the

allocated social worker, you will be expected to assess care needs in line with Sec 9 of the Care Act (duty to assess) and complete personalised packages of care to enable people to shape their own lives.

Pathway 1 community integrated team operates within a multi-disciplinary [MDT] setting with our health colleagues from Homefirst and Community Independence Service [CIS]. Therefore, every case your team, will have been discussed at the multi-disciplinary level in determining and identifying what service is most appropriate (Reablement or Long-term care , or Rehab therapy).

Taking a person centred and strengths-based approach will be necessary;

to enable choice and control from service users so they can flourish and achieve the best results from their care.

Pathway 1 community integrated team is a fast-paced team, which will require assessments to be completed within a 5-day time frame, and post assessment, transferred to our Complex teams for ongoing review. You will be surrounded by supportive colleagues and a manager who are dedicated to helping you do your best work, and you will be provided with

regular supervision.

We are looking for registered social workers with excellent communication skills, ready to build rapport with service users and their carers. You’ll demonstrate high levels of compassion and empathy. At the same time, you will need a good grounding in the legal issues surrounding safeguarding, the Mental Capacity Act and the Care Act to confidently assess needs and complete care plans. You will have knowledge of working within a multi-disciplinary setting.
