Good Afternoon, I am currently representing Tower Hamlets Council, who are offering an initial temporary contract for a period of 3-6 months with a view to be extended or offered a permanent position for the right candidate at a rate of £19.00 which is negotiable dependent upon experience. 25 Hours a week. We are looking for a Leaving Care – Personal Advisor this role will be: E14 2BG The right candidate will: * To manage a caseload, develop and monitor and review Pathway Plans of support and interventions in agreement with the service user. * Respond to requests for support and assistance, taking sufficient basic information about the needs in question to be able to determine the level and type of assessment required. * Participate in completing Pathway Assessments in a timely manner and in partnership with young people, carers, social workers etc. * Ensure that Pathway Assessments and Plans pay particular attention to individual strengths, areas for further development and action plan for moving onto independence; ensuring that the views of the young people as well as the relevant professionals are recorded explicitly in the assessments/plans. * Make appropriate assessments of individuals’ social care and accommodation needs in collaboration with health and other care agencies, taking into account the wishes of individuals and their families/carers. * Maintain case records and fulfil procedures and statutory duties in line with national and local policies, practices and procedures. * Work within budgetary limits laid down by Council policy and operate within the Financial Regulations of the Council. We require the following: * Direct work experience with children in care and leaving care, as individuals and in groups. * Knowledge of child care legislation including the Children Act 1989 and associated guidelines and regulations. * Knowledge and understanding of one or more of the following: * Education provisions, benefits and grants for care leavers. * Knowledge and understanding of issues impacting on Asylum Seeking and Unaccompanied children and young people. * Knowledge of accommodation issues faced by care leavers, welfare housing benefits, supported housing and permanent accommodation, education, training and employment in relation to care leavers. * Understanding of the processes of involving users in design and delivery of services. * Undertaking high quality and analytical Pathway Assessments, Human Rights Assessments, Personal Educational Plans, and the Pathway Planning process for looked-after children and young people.
