To work under the guidance and supervision of the stoma care nurses. CORE COMPONENTS • Provide optimum liaison and co-ordination of care for patients and carers between medical and nursing staff. • Provide support and reassurance to patients according to individual treatment plans. • Identify and maintain appropriate objectives in relation to role. • To work through and complete annual competencies to support HCA role • The post-holder will have responsibility to recommend the use of appropriate appliances to meet the individual needs of the patient, under the guidance of the stoma CNS'. • Be responsible for the provision of stoma care services under the guidance of the stoma CNS'. • Comply with Trust Policies and Procedures. • Be compliant with the staff appraisal system KEY RESULT AREAS Clinical • The post holder will have direct clinical involvement in and an understanding of caring for people with colorectal and inflammatory bowel disease and urological disorders. • Be aware of psychological concerns pertinent to stoma care, patients undergoing emergency surgery and cancer treatment to provide support and liaise with wider consultant team. • Promote a safe environment which facilitates the exchange of information between patient/carer/nurse and informed choice. • Facilitate advice to patients, clinicians and health care professionals. • Liaise with key personnel within the hospital and community, in relation to delegated group of patients and under the guidance of the lead stoma CNS. • Have an understanding of working collaboratively with other relevant practitioners and other key organisations (e.g. Ileostomy Association, Colostomy association and Chron's & Colitis UK)• Complete and maintain accurate patient records to meet the requirements of the Trust• . Communicate and feedback to stoma CNS'.• Practice within competency boundary seeking advice from the stoma CNS' where appropriate.• Agree on new competencies as identified and agreed by the lead stoma nurse.Administrative • Maintain appropriate and efficient systems of referral and review systems of documentation. • Organise workload to take account of priorities of care in meeting the needs of an allocated case load under the guidance of the stoma CNS'. • Collate reports as required to lead stoma nurse regarding issues relating to the provision of care, monthly reports and data collection and service audits. • Communicate effectively with colleagues in Primary, Tertiary, Specialist Care and other agencies as appropriate. • Attend relevant departmental meetings and discussions as required. • Provide a cost-effective service. Education • Identify individual learning outcomes for self. • Contribute and deliver education and training programmes for patients /carer and other Health Care Professionals. • Contribute to the service training programmes as appropriate within Hospitals and Community, under the guidance of the lead CNS.• Have an in-depth knowledge of Company and Competitor products. • Signpost health Promotion and education for patients and their carers.
