If you’re interested in joining Routes as a Case Manager in the North East, here’s what we’re looking for from you:Someone passionate about putting people firstOrganisational & time management skillsA good Understanding of CQC requirementsExperience in managing complex care packages with clinical interventions including, spinal injuries, acquired brain injuries, motor neuron disease, multiple sclerosisExperience of managing a team of healthcare workersYou will need to be a driver with access to your own car for workYour core role will include:To take ownership of a caseload of complex clients with clinical interventions including, spinal injuries, acquired brain injuries, motor neuron disease, multiple sclerosis and manage their care package holisticallyTo co-ordinate and lead the transition, start up, and continuity of all complex care packages; involving client, MDT; managing the clinical training needs of the care team, and maintainingappropriate skill mix to ensure safe and effective clinical practiceUpholding high standards in care and prioritising a person-centred approachPrioritising individual wellbeing in our care practicesBeing a responsive advocate for complaints and safeguarding, and encouraging improvementDeveloping comprehensive care plans and risk assessmentsProviding support for your care packages through observations and mentoring sessions, and regularly reviewsThis could be the start of an incredible journey together, changing lives for people with Complex Care needs in our community.
