Job Summary: The role of the Mental Health Support Worker is to provide comprehensive support to individuals with diverse mental health needs. This includes referring them to appropriate mental health professionals, coordinating with NHFT mental health services and third-sector teams, and navigating the mental health system to ensure holistic support and access to necessary additional services. Main Duties and Responsibilities of the Job: • Develop therapeutic relationships with service users and their carers to facilitate effective support and engagement. • Liaise and share information, as appropriate and in line with Consent to Share Information Policy, with others involved in the care of service users, ensuring sensitivity and confidentiality. • Engage service users actively in their agreed treatment plans and facilitate access to relevant services based on their individual needs. • Ensure service users' preferences and needs are central to care delivery, promoting their wellbeing and autonomy. • Support service users in maintaining meaningful relationships within their local community. • Meticulously follow care plans and promptly communicate any changes in service users' conditions or needs to the line manager. • Proactively contribute to the efficient operation of our service, ensuring the welfare and safety of those under our care. Person Specification: Essential Qualifications and Skills: • Strong proficiency in English, both written and verbal. • Certified in CPI/SIFL/PMVA – Foundation Level. • Proficient in maintaining accurate care plans and records in adherence to organizational policies. • Effective communication skills to support service users in daily activities and build rapport. • Experience working with service users, particularly those with mental health support needs. Desirable Qualifications and Skills: • Leadership abilities to assist in the onboarding and training of new team members.
