Contract: Fixed Term until March 2025
Hours: 37 hours per week, including some weekends and on-call shifts

We're seeking an experienced Support Service Co-ordinator to join a dedicated Care and Support team. This role centres on providing essential support to young families within a supportive housing environment, helping them towards independence. Based at a residential hub with nine flats and communal areas, this role is integral to building a thriving, supportive community.
Key Responsibilities:
Ideal Candidate:
You'll have experience working with vulnerable client groups, a strong understanding of adult and child safeguarding, and a supportive, non-judgemental approach. A valid driving licence and access to a vehicle are required for occasional outreach work across County Durham. The role requires flexibility, emotional resilience, and the ability to work independently when needed.

- Process new referrals, assess client suitability, and manage onboarding.
- Work closely with residents to develop person-centred support plans and hold regular progress meetings.
- Oversee property management, compliance, and health and safety at the site.
- Attend multi-agency meetings, providing updates and written reports on cases.
- Assist with income maximisation and help residents access welfare benefits.
