We're determined that with the right support, everyone can maintain or achieve some independence and we're proud to be working to make these ambitions a reality.This brief video provides an overview of the Community Support Worker role and includes insights from members of the ASC team - Watch Here.The RoleAs a Community Support Worker in an integrated team with the West Essex Care Coordination Centre, you will be undertaking assessments and reviews in line with our statutory duties under the Care Act to identify eligible needs and to then generate a person-centred and outcome focused support plan. You will be working in partnership and developing relationships with both internal and external colleagues / partners to achieve the best outcomes for Adults and Carers.This is a "community-based" role, so you can expect to be visiting adult in their home and the community hospital. You will be meeting colleagues and working in the office as well as working from home. The role covers 7 days per week and Bank Holidays on a rota basis.You can expect a varied role, supporting adults of all ages (18+). Working as part of a supportive and knowledgeable team that encourages your career development and growth. You will demonstrate a commitment to learning and development to support yourself and the team to deliver the best service possible.
