New Horizons Care has an exciting opportunity for a Part Time Clinical Lead to join our Team. 2 days a week (16 hours). Once a month we will require you to do an additional day to attend the management meeting held every alternate Thursday. Key Requirements: Must have a current Nursing PIN; and must keep this up to date via Revalidation. Main Duties will include: In partnership with the Services Development Manager, liaise with referrers and complete potential Client Pre-Support Assessments. Monitoring, recording and reporting changes in Clients conditions. Maintaining accurate and up to date detailed reports, records and documents. This will include updating relevant Tracking spreadsheets, providing completed clinical supervision records to the Registered Manager, updating clinical documentation related to Clients and their presentation and needs, keeping training records updated, etc. Liaising with Clients, their families and other MDT professionals. Identifying training needs; and conducting clinical training for Support Workers on an individual and group basis. Leading clinical training sessions such as PEG, Medication, Epilepsy, Buccal, Tracheostomy, etc. Completing weekly diary in advance so that the Registered Manager and Director are aware of your movements. Conducting clinical supervisions and competency assessments. Creating and updating clinical care plans and MAR charts. Completion of relevant audits. Helping to prepare for CQC and other inspections. Monthly MAR chart audits. Other Key Tasks To provide clinical guidance and support to all Support Workers employed by New Horizons Care. To assist the Registered Manager in being compliant with all regulatory standards i.e. Care Quality Commission Fundamental Standards, Environmental Health, etc. Have a good understanding of the Health and Social Care Act, Mental Health Act, Mental Capacity Act (MCA), Deprivation of Liberty Safeguards (Best Interest), etc. To work with the Registered Manager during Care Quality Commission inspections, Clinical Commissioning Group inspections and Council Contract Monitoring reviews. To make improvements in the quality-of-care provision, following discussions with the Registered Manager, and following the results of internal audit processes, CQC inspections or Council Contract Monitoring reviews. To assist in ensuring that the service operates at a consistently high level, thereby demonstrating to the Care Quality Commission and the various Councils that Clients are: 1) safe and well cared for 2) receiving good personal care 3) the treatment of disease, disorder and injury To attend relevant courses and training, in order that the Clinical Lead is always up to date with current best practice; and the Clinical Lead is to ensure their CPD is up to date as well. To participate in the Senior Management Meeting whenever these take place, partake in discussions in this meeting and to provide clinical updates. Thereafter to complete relevant actions and tasks which have been delegated to the Clinical Lead. To assist in ensuring that all Clients are kept safe at all times. To make Safeguarding referrals in conjunction with the Registered Manager; and to make enquiries with the local Safeguarding Team related to possible Safeguardings if unsure. Liaising with hospital staff to ensure safe discharges for new packages. Always act in an open and transparent manner, irrespective of whom the Clinical Lead is dealing with (i.e. Director, Registered Manager, Staff, Clients or external personnel).
