Continuing Healthcare CHC Case Manager

Role:  CHC Case Manager Location: Lincolnshire Full-time A great NHS Trust in Lincolnshire is looking for an experienced RGN/RMN/RGLN to join their growing CHC team. Main Duties: Support the Locality Lead in the development of a range of services required for individuals who qualify for packages of care Case Managing CHC individuals Communicate information, risks, issues and dependencies, including briefings and reports. Procide professional nursing advice to acute hospital trusts, care providers and social sercices with regards to Continuing Healthcare. Minumum Qualifications: Registered Nurse RGN, RMN or RNLD or Allied Health Professional with qualification Knowledge of NHS and social services guidance Strong knowledge base around safeguarding children and Adults Evidence of teaching/mentorship Clinical knowledge base in order to ensure comprehensive holistic assessment for continuing care Must have a UK driving Licence as travel county-wide is required at times.
