Hammersmith and Fulham Housing Support and Inclusion Service is the largest generic floating support service within the borough. Support and assistance are provided to vulnerable adults living independently within the community. You will be managing and supporting housing floating support workers to deliver a floating support/outreach service, with a primary focus on housing and benefit-related support for vulnerable adults aged 16 and over who are referred to the service. A day in this role looks like…. As a Floating Support Caseworker at Hestia, you will play a crucial role in our Housing Support Inclusive Service. Your contributions will directly influence the lives of our service users, guiding them towards a path of empowerment and independence. A day will never look the same and some of the things you can expect to do/be responsible for are: To be responsible for the support and well-being of a group of key service users linked to local mental health services in Hammersmith and Fulham using holistic, person-centred support planning with SMART targets using the recovery approach to enable them to increase independence, maintain their accommodation and enjoy a good quality of life To visit service users in a variety of settings, including within their independent homes, the community, supported housing services and hospitals. To devise effective interventions for service users who are ‘hard to engage' To enable the service users to access medical services and the local community mainstream services To liaise effectively with and work alongside key partners from relevant services across Hammersmith and Fulham, including community mental health teams and local authority housing teams. Coproduction work with residents in partnership with providers and commissioners to help shape the new model, involving discussion around satisfaction with supported housing and the homeless pathway and what could be different. Working with the mental health & single homeless supported housing services to understand gaps in MH clinical and other wrap around support once people have moved on and to work with partners at West London Trust and H&F council to address these gaps. To ensure that allocated service users have a needs & risk assessment and SMART support plan; which are reviewed at agreed frequencies To minimise unplanned and emergency hospital admissions for mental health relapse. To formulate, implement and review support plans with service users and associated professional participation To encourage users to have on-going engagement with existing networks including faith-based groups and informal sources of support Why Hestia? To excel in this role, you should possess: Experience of working with people requiring support related to mental ill health and complex needs Experience of delivering support to people within a community/outreach setting Ability to support service users to achieve their goals & aspirations Excellent knowledge of, issues related to signs, symptoms and treatments of mental illness; ability to assess service users declining mental health and take appropriate action Knowledge of health and safety relevant to the role including working within a community setting Knowledge of key working, person centred support planning systems and the recovery approach An understanding of equality and diversity and how it applies to the job role and the importance of promoting service users' rights, privacy dignity and choice Knowledge of welfare benefits entitlements appropriate to the user group Knowledge and understanding of safeguarding issues and ability to address them appropriately Our commitment to Equality, Diversity, and Inclusion Our services users come from all walks of life and so do we.
