Practice Nurse

Job Title: Practice Nurse Location: Darlington, UK Job Type: Full-Time/Part-Time Salary: Competitive, commensurate with experience Job Summary: Well-established medical practice in Darlington dedicated to providing patient-centred care and are seeking an experienced and committed Practice Nurse to join our health care team. The successful candidate will play a crucial role in delivering high-quality care to our diverse patient population. Key Responsibilities: • Deliver comprehensive nursing care, including chronic disease management, wound care, immunizations, and health screenings. • Conduct patient assessments, develop individualized care plans, and support the provision of patient-focused care. • Collaborate closely with GPs, healthcare assistants, and other members of the multidisciplinary team to ensure seamless care. • Provide patient education and health promotion advice, empowering patients to manage their health and well-being. • Maintain accurate and up-to-date patient records, ensuring compliance with all relevant guidelines and protocols. Qualifications and Experience: • Registered Nurse (RGN) with a valid NMC pin. • Previous experience in a primary care or community setting is highly preferred. • Proficiency in managing chronic conditions such as diabetes, asthma, and COPD. • Excellent communication skills and the ability to work effectively as part of a team. • Competency in using clinical systems and electronic patient records.
