Adult Case Manager

Adult Case Officer Westminster Contract £30 Umbrella Per Hour The role: We are looking for a Adult Case Officer for a role in Westminster. The Case Officer will conduct visits to residents homes providing information, advice, assistance and advocacy to support service users who wish to repair, adapt or improve their home. To identify their needs though review of referral information and to discuss the options for addressing these through the Home Improvement Agency and other agencies. The case officer wiill be the main point of contact for the service user throughout the process to support them to follow through with their chosen options and maintain communication, demonstrating sensitivity, flexibility and sound professional judgement. The Candidate: Sound Local authority experience. Knowledge of equipment, Major and minor adaptations. 2 Experience of functional assessment experience of working with people with disabilities/vulnerable people. Understanding of Major Adaptations.
